If you have any questions about completing this application, please call the Allen County Probate Court at (419) 227-5531, select #2 for the Probate Clerk's Office, and then select #3. You are also required to appear in person at the Court, located at 1000 Wardhill Avenue, between the hours of 8:30-12:00 pm or 1:00 pm - 4:00 pm, Monday - Friday (during regular business hours - ** licenses will not be issued after 4:00 pm **) after you have submitted this application. If you do not appear within 30 days to complete your application, it will be deleted, and you will need to resubmit an application. If you DO NOT HAVE A UNITED STATES SOCIAL SECURITY NUMBER, please put 000-00-0000 as your SSN #.
